If you are considering offshore medical billing, India and the Philippines are the two countries you will hear about most. Both have large, experienced healthcare outsourcing workforces. Both can be safe and effective. They are strong at different things, and the right choice depends on what work you need done.
A note on bias: DrBillerz is based in Bangalore, India. We have tried to be fair below, and where the Philippines is the better fit, we say so.
The short answer
India is usually the stronger choice for the technical side of the revenue cycle: coding, charge entry, claim submission, denial management, AR follow-up and credentialing. The Philippines is often the stronger choice for patient-facing voice work, such as patient calls, scheduling and front desk support. For most practices, the bigger factor is not the country. It is how well the vendor tests, manages and oversees the people doing the work.

Where India is usually stronger
India has one of the largest pools of trained medical coders and billers outside the US, built over more than two decades of US healthcare outsourcing. Many billers have worked across multiple EHRs, clearinghouses and specialties, and certifications such as CPC are common. That depth matters most for work where accuracy and payer rules decide the outcome: coding checks, denial analysis, appeals, AR calls to payers and credentialing paperwork.
Where the Philippines is usually stronger
The Philippines built its outsourcing industry on voice support, and many teams are very comfortable on calls with US patients. If most of the work is answering patient billing questions, taking payments over the phone, scheduling or insurance intake calls, a Philippines-based team is often a very good fit.
What matters more than the country
HIPAA applies the same way in both countries: the vendor is your business associate, and you need a BAA, access controls and oversight. The risks that actually hurt practices are the same everywhere:
- Untested staff. Ask how billers are tested on your specialty and EHR before they start.
- No manager. Ask who reviews the work every day and who you call when something goes wrong.
- No visibility. Ask what numbers you will see and how often.
- Shared logins. Every biller should have their own login that you can switch off.
- Turnover. Ask what happens when your biller leaves, and how fast a trained backup takes over.

A simple way to decide
- Your problem is denials, AR, coding or credentialing: start with a team strong in technical RCM, which usually means India.
- Your problem is phone volume and patient communication: start with a voice-first team, which often means the Philippines.
- You need both: many practices use a technical billing team for the back office and a separate voice team for patient calls.
