Physical therapy practices bill many small, timed services per visit, often to the same patient several times a week. • Timed-code units. Calculating billable units for timed codes correctly, including the Medicare 8-minute rule, and documenting time to support them.
• Therapy modifiers. Using the GP modifier for physical therapy services, the KX modifier when Medicare therapy thresholds are reached and medical necessity is documented, and the CQ modifier for services by physical therapist assistants.
• NCCI edits. Knowing which code pairs need a distinct procedural service modifier, such as 59 or the X modifiers, and when using one is not supported.
• Plan of care and certification. Tracking certification dates and required progress reports so claims are not denied for missing documentation.
• Authorizations and visit limits. Tracking authorized visits and plan limits, so treatment is not delivered after coverage runs out.
• Multiple payers. Medicare, Medicaid, commercial plans, workers’ compensation and auto claims each have their own requirements.
